Insurance Information
We are providers for the VSP Signature Plan (Vision Service Plan) and the original Medicare plan. Dr. Lundy is in the process of joining the following plans: United Healthcare, Blue Cross Blue Shield of Georgia, and Cigna.
VSP -- All co-pays are due at the time of service. You will receive a Well Vision Statement at the conclusion of your visit which explains your out-of-pocket expenses. If you have a question regarding your VSP plan, you may call VSP's member help line at 1-800-877-7195.
Medicare (original Medicare) -- As a Medicare insured, you are responsible for the annual deductible of $135 and any amounts not paid by your supplemental insurance plan. Medicare pays 80% of the approved billed amount. The remainder is billed to your supplemental plan. You will not be asked to pay deductibles or copays at the time of service. We will bill Medicare and your supplemental plan, and if there is any unpaid portion remaining after insurance pays, due to deductibles and coinsurance amounts, then you will receive a statement.
Medicare Private-Fee-For-Service Plans -- We will file with your private-fee-for-service plan with a medical diagnosis, provided that the plan approves benefits that are equivalent to the original Medicare allowable amounts. Any fees wich are not approved by your plan, but would be approved under the original Medicare plan, will be your responsibility. Your plan co-pays, deductibles, and any out-of-network balances will also be your responsibility.
If you are not a member of one of these plans, we would ask that you pay for your services on the day of your visit. We will provide you with a walkout statement, which you can then file with your insurance company for reimbursement.
